All new patients must be referred to our office by another physician.  For a surgical consultation the patient must have current diagnostic testing such as an MRI or CT scan and should be brought in at the time of the patient's visit.

As the referring doctor's office you should provide...

1. Demographic data, MRI, Myelogram, Discograms, CT, EMG/NCV study. Laboratory, and treatment modality reports
2. Insurance Information
3. Workman's Compensation Information (if applicable)
4. Last Dictation and current H&P if available.
5. Any previous medical records, especially tests results ( i.e., EMG/NCV, Bone Density, Pain Management documentation)

Our regular office hours are 8:00 to 5:00, Monday through Friday.  Our phone number is (817) 878-5333; fax (817) 878-5334.  When you have an emergency referral outside regular office hours, call the same number and our answering service will locate the neurosurgeon on call.

Please encourage your office to help referred patients understand the importance of their bringing all pertinent information regarding previous treatment, including all medications.  Any diagnostic studies are particularly critical.  We cannot effectively evaluate patients without these.
